Applying for C&A positions, Training Verification form?

Hi,
I was wondering when we are applying for Child Psych, I understand that there is a form that we need to attest our eligibility to sit for the boards and the amount of training we have would suffice to be eligible for C&A fellowships. However, on ERAS, can we see this form? My program coordinator told me it had been uploaded, yet I can only see my LOR's on my end. Is this a form that is not viewable to us as applicants?

Typically the form was uploaded at the end of the PD letter, which if I remember correctly is with your regular LORs. Just to be precise though you aren't attesting your ability to sit for the boards, your program is attesting that you have done xyz required rotations to enter CAP fellowship and still meet all the requirements.

Thanks, so is it a separate letter that I will have to request through ERAS? My PD Has a letter already but it was uploaded without it attached.
 
So ask your PD if they uploaded the training verification with their letter. If not they will need to reupload their letter with the training verification in the same document.
