Applying for programs while being abroad on a Fulbright

This is such an out of the ordinary question that you would probably be best served just asking the program directors of several programs you would be interested in going to.
 
I doubt anyone will take you into their program without an interview.

Therefore, I agree with the above poster and you should contact programs this year, and try to arrange some interviews. Explain your situation. Perhaps someone will sign you outside the match for the 2010 training year.
 
I have had friends in this situation. The answer is no-- you are required to travel back to the States for interviews. If you can schedule them all in, say, a two-week period around Christmas or Thanksgiving, that is quite in keeping with the overseas residency requirements of the Fulbright and you'll be fine. It's my impression that IM and peds programs tend to be quite flexible in this regard and offer multiple interview dates. Just get everything in super-early so you'll have the leverage to schedule interviews when you wish.

why don't you call up your top 3 programs' directors and tell them your situation. Perhaps you could interview THIS year and they'd give you a 2010 spot. You are somewhat in the driver's seat when you are talking about peds and IM, assuming you did fairly well in med school and passed all your USMLE's, etc. It's only hard to get a spot at the super picky IM and peds programs...residency match = a numbers game and there are just so many more people and so many fewer spots in some of the other specialties.
