Does anyone know how this works? On AMCAS, a bunch of schools basically say that it's restricted or you need to clear it with the school first? Can I just say I want to do a dual degree on the application? I have very little clue. For reference, I'm mostly applying to top programs.

The process varies from school to school. There's absolutely no way to give advice about how it works because every school does it differently. You need to look at each school's website and figure out what the requirements are. Generally, if it's restricted then you need to get permission to apply MD/MPH, and just selecting that you want to do a dual degree won't be enough or won't get your application shunted to the correct committees.
