I am applying to a summer program and there is just only one section for extracurricular actitivities, and I wanted to know if putting that I worked as an office assistant in the premed office was okay. Since one of my hobbies has been learning to play the piano, I wanted to know if it was okay to put that on my application. I take piano classes at the college I am attending, but I have been playing the piano for three years.

I'm tempted to say piano if those are your two choices. It shows your personality more than being an office assistant does - premed office or not.

I am applying for a premed summer enrichment program that offers research, clinical experience, and lectures on anatomy and physiology.

Your office job demonstrates that you can behave in a business-like manner and are responsible and trusted. Dedication to piano for three years (theoretically) shows you can learn a new skill, maintain discipline over time, and have manual dexterity. Either would be suitable to include.
