applying to two specialities as a backup

hi,i am an IMG with scores 234 and 218 in step 1 and 2, respectively- all exams cleared on first attempt. my cv is entirely focused towards pediatrics
(research, usce etc). however, I do not meet the step 2 cut off for most peds programs so I'm considering applying to one more specialty as a back up. I was never really in favor of doing that but even peds has become really competitive for imgs. (I didn't match last year).

in all honesty, I like family medicine as well. specially programs in rural and underserved areas do interest me a lot. but im afraid my cv is too peds oriented and PDs might perceive that I'm just applying to fm as a backup. I really want to match this year. I like child psych as wel but worried abt the same thing - cv has nothing that shows interest in psych. any advice/ input will be appreciated
 
There's no easy answer. You could:

1. Try to add things to your CV to make it more FM/Psych oriented. Do observerships in those areas. Do research in those areas. Unfortunately, you probably should have started that back when you didn't match last year, since there isn't much time left now.

2. Be honest. Write in your PS that you were initially interested in Peds, but that you didn't get in. Instead, you're now applying to FM because it will allow you to treat both kids and adults. Or psych so you can ultimately do child psych.

3. Do neither and hope someone decides to interview you anyway.
 
I vote for option 1. Your scores are super good for FM. I wouldn't mention other interests in my personal statement, keep it straight forward FM. You can always answer the PDs about your peds history that this what was available. This trick really works in my experience with IMGs. They do understand the limitations IMGs face when looking for USCE or research. Do a wide application to FM programs but don't give up on peds, I think you still have good chance there too.
