What is the latest someone can officially apply to MD/DO? I know its best applying early, but what is the latest?

it really depends on the school. amcas has a specific deadline in the fall, but most individual schools that use amcas also have secondary applications, some of which are due in the spring.

again, these amcas deadlines do not include the secondary applications.

foreign medical school run on totally different schedules, so you'll have to look at the specific school websites to find out when applications are due.

All schools will have their own deadlines for submitting the primary and secondary application. When you add a school on AMCAS, it'll tell you the latest date the school will accept apps.

The deadlines are:

Oct 1, 15; Nov 1, 15, 30; Dec 1, 15

You can also sort by deadline on AMCAS

Yes. You can start working on it any year, though unless you submit it it doesn't save your information for the next cycle.
