I have a marijuna IA from freshman year but also two gaps in my education. I took 1 year off between freshman and sophomore year to recover from an illness and have a medical leave a year after coming back because my illness came back which left a semester of W’s on my transcript. Since then I have recovered and have good academic standing with a good gpa. I know the IA is not good but how bad is having two a gap in education and a medical leave ? Do I still have a shot at top schools or any md school at all?

Your MJ IA won't eliminate you from the running. Taking a year off and withdrawing from one semester due to medical reasons won't either. At the end of your secondaries you'll typically be asked "anything else?" That's your opportunity to briefly mention this. Just my thoughts.

It's a common pre-med delusion that your UG education has to be in 4 years straight and right from high school graduation.

The IA is trivial. Own it and you'll be fine.
