Asking a Dentist for a Rec

I'm planning on asking the dentist who I shadowed for a rec...is there any way to not make it awkward haha? He likes me, but it's hard to say "hey...i know you've got work, kids, and a life, but can you help me get into dental school?"

Also, I have no idea what I should provide for him. I'm going to give him my resume (activities, research) and tell him to just talk about his/my experience shadowing. Any tips?

Remember that the dentist knows that the reason you are shadowing him or her is because you want to go to dental school (if there is any other reason, please let me know). Therefore, he or she is probably expecting you to ask for a LOR. First, let the dentist know that you really appreciate his or her help in not just the experience, time, teaching, but also for their help for a good LOR. Believe me, he or she probably did the same thing when they applied. I think what you mentioned to provide your dentist is right on. If the dentist is really busy, offer to write a draft and have he or she edit it for you prior to sending it out. Hope this helps.
 
the dentist can write about what qualities he's seen in you and/or he can talk about your motivation and dedication. it doesnt only have to be about outside stuff, adcoms like to see the personal characteristics YOU possess that would help you get through dental school and eventually a great dentist one day!

Ditto. The more specific the LOR is about you in terms of your personal as well as academic qualities, the better the LOR.
