What do I need to give my attending doc when I'm asking for a letter of rec? I have my CV ready and an envelope. Anything else I need to include when asking for a letter?

$50 gift card to their favorite restaurant. Just kidding, that may not go over well with some attendings.

I usually put a small note in their expressing my appreciation for them caring enough to take time of their busy schedules to help.

The fastest way would be to have a written letter on a hospital letterhead with enough space for him to sign his name.
 
Besides my CV, I also included a copy of my transcript and my personal statement in the packet of info I gave to my letter writers. I had my first LOR writer request all of this and I just sent the same stuff to everyone after that. Everyone seemed to appreciate it.

Just a thought.

It's a helpful reminder to me of the preferred format for the letters and it reminds me what information to include (besides all of the glowing recommendations, fanciful adjectives and excessive hyperbole).
