I doubt the problem is in the contract (it rarely is). Its more likely they have to offer that much money because they are having troubling finding candidates for whatever reason.

How long is that salary guaranteed?

Dunno. You haven't told us much more than the money and the hours. Where is it? Who are you working for? Is it a short-term guarantee, or a long-term salary? Is there any production/bonus? What's the upside? That's all as important (if not moreso) than the initial starting pay. In most cases, salaries are for suckers, and high-paying jobs are high-paying for a reason (e.g., beware).

a 4 day work week could mean that you have 32 patient hours (4 days x 8 hours per day of patient time) and 8 hours of admin time.

There's a LOT more to a great job than just a high salary. There's a very good reason that job is offering over $100k more than the average starting FP salary, and it's not that they feel like being generous.
