What is the authorship order like for rad onc clinical journals? Is it like basic science journals where the first author is the grad student/post-doc who did most of the work while the last author is the PI? If one was given a broad idea, but did all the data gathering, analysis, and wrote the paper, which place in line should that person be? Thanks a lot!

For your run-of-the-mill retrospective study, you are correct. The first author is usually a medical student, grad student/post-doc (for physics), resident or junior faculty member. The last author is generally the mentor of the first author.

For publications of prospective data the first author is generally whomever did the most work (trial design, analysis, writing). For larger trials, like Phase III data, the first author can be quite senior.
