Immunology states that type 4 hypersensitivity is cell mediated and does NOT involve antibodies.

How can we, then, explain the presence of anti-GAD and anti-islet cell antibodies in type 1 DM or anti-thyroid peroxidase antibodies in Hashimoto's thyroiditis?

Think of them as markers of the specific disease, but they are NOT directly involved in the pathogenesis of the disease, I believe.

Celiac disease is another example (a type IV HS reaction, but anti-TTG, anti-endomysial, and anti-deamidated-gliadin.
