To answer your questions:
1. You pay tuition at your own institution.

2. Many away departments will have lists of places for visiting medical students to stay but you are responsible for your own housing. Once you have arranged an away rotatation, immediately contact the sponsor of that rotation for finding housing options. Some places (like Mayo Clinic in MN) have housing for visiting students at a reduced rate.

How typical is it to do sub-I's at other institutions?

Likewise, what makes a sub-I different than any other M4 clerkship? Responsibility? Workload? My impression is that they are only available in things like medicine and surgery, but I want to say that somebody told me once they you get paid as a sub-intern. That seems contrary to our mission to remain servants as med students.

Has anyone ever heard of a state school letting you do a perfunctory "rural" rotation (M4) outside of the state?

I agree w/ the above. If, however, the school you're visiting provides NO help w/ housing (like the one I went to), then you might try using something like Craig's List. They usually have lists of temporary sublets for most areas in the coutry. That's what I used and it worked out well....just make sure you get a contract w/ whoever you're subletting from!
