So, it is written on question's 4368 explanation that NO focal point tenderness is a red flag for back pain... When you get through question's 4691 you read that focal point tenderness is seen in spinal cord compression due to metastatic disease (or another causes).

Is this contradictory or am I missing something?

note: obviously I'm not a spine expert

The explanation does not say that "no focal point tenderness" is a red flag. This patient happens to have no focal point tenderness and also has red flags as listed in the explanation. You've misread the explanation.
