Ok,

In galvanic/voltaic and electrolysis, unless otherwise stated, are we to assume that the electrodes are immersed in water?
 
You can assume the anode is immersed in an anodic solution, and the cathode in a cathodic solution--this will almost always be an electrolytic solution (not just water) containing the appropriate ionic species for the redox reaction to occur and continue.
