I'm planning on doing a sub-i during the first few months of 4th year. When is the best time? I'm currently planning on doing it during the first block, to get it over with. I was also thinking it might be to my advantage to rotate with the new interns because attendings might be in more of a teaching mode during those early weeks in the hospital when all the teams are new. However, we only have a weekend off in between third and fourth year so I'm worried about being burned out. Thoughts?

I think most people try to do their home rotation first so that they are able to shine more on away rotations.

Also, I know a couple of PDs who recomend that interested students NOT rotate at the same time as the new interns because they are so busy trying to orient the new interns that it takes time away from the students.

Maybe, if you would really rather stay home that go somewhere else, it might be benificial to do an away first. I don't think that's the usual approach though.

I felt the best time to do my subI was a month into 4th year. I took 2 wks off to take step II and did a slack geriatric rotation before starting my hardest subI. Then I was rested but still pretty sharp after studying for the step. IMHO, the earlier the better.

The million dollar question is: "what specialty are you going into?"

If you are going into medicine (medicine, MICU), surgery (GS, SICU), surgical subs (GS, SICU), anes (medicine, GS, M/SICU), you probably want to do your sub-I early so that it will be on your Dean's Letter for ERAS. Otherwise, you can do it later when your grades doesn't affect your ERAS any more.
