Big Mistake on AMCAS/Secondaries - Need Help

I made a very big mistake, and I am not sure that I can fix it. I withdrew from a semester several years ago, but there was a mistake with the withdrawal. As a result, I received multiple failing grades for the semester. I did not return to school for several years. I just realized last nigh when I was looking over my transcript that because of this semester I have a probation notation on my transcript. I was never contacted about this, and when I returned to school, it was never mentioned that I was on any kind of academic probation. Last year when I applied, I contacted the Dean's office to see if I had any institutional actions on my record, and I was told that I did not. Therefore, I answered no to the Institutional Action question on my primary AMCAS application last year and when reapplying this year. I had previously never noticed the probation indication on my transcript, and I am freaking out. I have already completed my secondaries for this cycle, and I have answered no on all of the Institutional Action questions and any questions about academic probation. I honestly did not think that I had ever been on academic probation, and now, I believe I may have ruined my chances of ever becoming a doctor. Is there any way I can fix this problem? I don't believe I can fix it on my AMCAS application, but if there is a way, I would love to know how. I also don't believe I can edit my secondaries either. Should I contact each school individually and explain the situation? Is there any way for me to contact AMCAS to have this fixed? I have no idea how to proceed, and I want to fix this before it becomes an even bigger issue. But, I am honestly at a loss. Any information or help that anyone can provide would be really appreciated.

I would contact each school individually, AMCAS will not fix this. Worst case scenario is schools thinking you're trying to conceal something.
 
Thanks for the reply. That is what I planned on doing. I don't want to appear like I am trying to conceal anything at all. I honestly didn't even realize it was there until my wife said something about it last night. I am pretty new to this site, and I just joined to try to get some advice. Are there any people on here that I should try to contact in particular who might have some insight? Thank you again for your response.

First, I am not sure whether academic probation is even considered an Institutional Action (typically disciplinary oriented), so verify this first.

Also, I assume you have already thoroughly explained your withdrawal and grades in your narrative. As such it might be all you need to since it is pretty clear if you have F’s on your transcript. Nothing additional may be needed.

Academic probation is an institutional action... the school put you on "probation". It should have been reported. You weren't aware of the fact that you were on probation because you left school and in that instance you wouldn't have received further correspondence from the school about still being on probation or having rectified your academic standing and no longer being on probation. All very understandable from a school administrator point of view. It is also rather obvious given your GPA at some point dropped below a certain threshold.

Send an email to each school where you have an open application and state that you wish to bring to the committee's attention a situation that you had overlooked when you submitted your AMCAS application. Specifically: you were on probation at Littletown College after the [X] semster but that you did not enroll during the subsequent term and eventually resumed your education at Bigtime University in [season, year].
