Really the best person to ask is your attending physician, so you will know the details that need to be included in the note according to their standards.

Definitely do what is above, but as an interesting aside I had a similar request and used an AI called ChartNote to turn my notes into billable notes.

It wasn't perfect, but it sure was easy. And since I was writing something billable that I wasn't getting compensated for I went for easy.

Depends on specialty, context, other circumstances, etc. I mean, if you write "We saw the patient together and discussed plan with patient" as your entire note, that might be billable, but probably not at a level your attending would appreciate.

Very generally speaking, the specific things they teach you to ask about/document in your notes in med school are because they are components of billable notes.
