In a general ENT clinic, it's very common. However, there's usually no camera/video system so you may not be able to see much until the wax is out.
In an academic setting, it's less common. Nobody waits 3 months for an appointment with a neurotologist to get their ears cleaned. However, there should definitely be some patients needing wax removal at an academic ear clinic (needs to be removed before hearing testing or for an adequate exam). And other subspecialists (H+N, rhinology, etc) usually don't spend much time looking in the ears.
