C2 rx decision...please help!

Hi,

Please give me some advice on C2 rx prescriptions...2 cases..

1) If a doctor writes rx for percocet 5/325 1 q4-6h prn pain #1 week supply. What would you do? (would you calculate yourself as 6 tabs (1q4h-6h)x7=42???)

2) Concerta rx came with 3 refills on a prescription. Would you just fill it with 0 refill or ask for a new rx?

1. I would call dr and ask for specific qty. in Texas, only things we can't change is Name, name, name, and date, so changing qty or sig is perfectly fine.

2. I would ignore the refills obviously.
 
For any controlled meds there must be a specific qty and I believe it must be written out also in words.
 
is this retail or ltc?
 
For 1st one just call and ask for a #

Second case, just ignore the refill... Don't need a new rx
 
#1 gets sent back, can't add a qty and a days supply is not a qty esp when PRN. In this case, you're not changing a qty, but adding one
#2 omit refills, or if you want to be nice call the office and get it changed to a 90d supply with Code B
 
If the doctor didn't indicate a quantity send it back. Don't bother calling if it's illegal in your state to alter a CII.

For the last one I would ignore the refills. Tell the patient that CII's cannot have refills and they'll need to get another hard copy from their doctor for each month.
 
If you've never seen these people before, call the office. And not the number on the script, the number in your database.

#1 goes back, tell the MD to write it correctly
#2, fill it, ignore the refill
 
Any refills on a CII or more than allowed by law for a III-V should always warrant a call to the doctor. Follow state and federal laws. If your state says you can't make ANY changes to a CII then refuse the script.

It doesn't *always* warrant a call. There are doctors in my area who routinely put 3 refills on all their RX's, including their CII's. They have been practicing for many years and are well aware of the law, but apparently can't break their habit. I just ignore the refills and fill the script. When they write 12 or PRN refills on their CIII's -CV's RX, I also ignore those and give the pt 5 refills. I would only call for an out-of-area prescriber that I wasn't familiar with.

Back to the OP--"1 week supply" on a PRN, any PRN, but especially a controlled doesn't work. Either send it back with the pt, or if allowed by your state, call the office to get an exact, numerical quantity.
