I just finished PGY1 and will be applying for California license. The forms are crazy long. For those who have already gotten their license, or are currently applying, can you provide some insight? Thanks.
1. L1A-L1E:
-on L1A, there's only 3 boxes but 4 exams (1,2ck,2cs,3). Do I leave out step 3?
-So I fill out L1A-D, then take L1E to notarary public to get it certified. Is this the package I send to CMB w/o any other documents?
2. Live Scan Fingerprint.
-I get my fingerprints taken and then mail the completed form to MBC (Medical board of cali) separately form L1A-L1E or just add it to my package?
3.L2 (Certificate of Medical Education) w/ Medical School Transcript and Certified Copy of Degree
-Do I mail L2 to my medical school and request that the Send copy of my diploma and transcript directly toe CMB?
-Do I have to provide a pre-addressed stampled envelope for mailing it to MBC or will the schools do it?
4. L3A-3B: Certifcate of completion of acgme post grad training
-Fill out Part 1 and give it to program directory to sign and mail directly? Or does he give it back to me and I mail it w/ my application?
5. L4: Certificate of Current Postgraduate Training enrollment
-Fill out and give to program director to sign and mail directly? Or does he give it back to me and I mail it w/ my application?
6. Fees
-Its $51 for fingerprint processing + $442 for application fee + $402.50 initial license fee = $895.50 made out to MBC?
7. Official Examination scores mailed directly to from the reporting agency.
-I go to www.nbme.org and click on Request Documents and see this message: Based on our records, you have either registered for or taken Step 3. You must go to the Federation of State Medical Boards to request your score document.
-How can I arrange for Step1,2ck,2CS and 3 to be sent to Cali Medical Board? I cant find the request section of FSMB
