My school has a Health Professions office and they told me they are taking care of all the recommendations. So they want me to get 4 letters from professors and dentist to them and they are going to organize for me.
But, I also see the "Evaluation Form" available on the AADSAS application too.
I really want to know how this works.

If my school's health professions office wants the actual recommendations, then how am I going to use them or like.. upload them on AADSAS application?
Do I have to send them separately? Both from school and from online? If that's the case then wouldn't my professors and doctor have to write the letters twice?
Please tell me about the process...

Also, what if my professors want to send letters through online? (electronically submit)
Then, I would not need Health Professions Office to take care of the online letters. correct?

Please help!

I sent my LORs to my health committee. They interviewed me and compiled my letters. I gave them a transcript matching form (1 form from ADEA application). They will send out a packet of all of my letters using that one form. Many schools like these committee letters (or so it says on their websites)..so it is advised to use a committee if you have one.

So steps:
1. Get your professors to send letters to your committee
2. Interview with your committee, fill out their requirements
3. Give them a transcript matching form (or if they do email, then set that up)
4. They will email/mail every LOR they received on your behalf.

Going the committee letter route helped me because I submitted 5 letters all together.

4 science professors (including research, ECs) and 1 general dentist (separate from my committee)

ADEA allows: 1 committee letter + 1 other letter

OR ADEA allows: 3 regular letters (could be 4, I forgot)

Hopefully that answered your Q.
