do GCMPs apply to?

1_ pharmacies that re-package and label dxs for other members of their chain

2. hosp pharmacies that re-pack and re-label dx for in-hosp use only

3... indiviudal rphs that re pack and re label dxs as members of an
informal buying droup

4. hosp pharmacies that re-pack and re-label drugs for other hosp

1 trade name

2 name and quan of all active and inactive ingred

3 adress of man . packer dist etc

Currrent Good Manufacturing Practices apply to all noncompounding activities that a pharmacist may do. The answer is all of the above in my state, because to compound, I need a physician-pharmacist-patient connection through an Rx. The activities described above are considered manufacturing in my state.

On the second question, 2 and 3. Item 2 dates back to the F&C act of '08! Item 1 need not be present, "acetaminophen 500 mg" is just as good as trade name "Tylenol". Item 3 is necessary for Poison Prevention Act at least.
