Cant reapply after eligibility period extension

The point is not to let your eligibility period expire without doing anything about it. You can apply for an extension while you're in your current eligibility period, which I think results in a rather large fee but not as much as it would cost to completely re-apply for the test again.

Unless you failed your attempt. Then yes, it looks like you have to wait 4 weeks between eligibility periods.
 
Is it the same if you do not take the test during the one time extension of the eligibility period?

I'm not familiar with this ECFMG thing and if they have different policies. My understanding is that if you schedule a test and don't take it or fail it, you have to re-apply in full, and that is when you have to wait 4 weeks between eligibility periods.

I don't know what happens if you don't take the test during your extension of the eligibility period. At that point you would have had 6 months of eligibility. If you need more than that, I'd assume you would have to re-apply in full.
