I am a PGY-2. I want to pursue CAP fellowship. When should I start for the application? Is it too hard to get a position in the South-West? How should I prepare?
 
Typically, programs don't start taking applications until the summer before you matriculate. As far as I know, the majority of programs go through the match. The match comes out in early January of the year you'll start.

If you're just starting your PGY-2 year, I wouldn't worry about the process yet. Just focus on your adult training. You can make sure you have some exposure to child psych sometime this year so you can get letters and have the ability to demonstrate you know what you're getting into. I suppose you could also try to find an interesting case and try to publish a case report. But to be honest, it's really not hard to get into a child fellowship and it isn't necessary.

Can't speak to southwest programs.

it isn't hard to get a program anywhere. Some "top" programs even have difficulty filling with good candidates.

why the prejudice/grudge-revealing portrayal with the italics? A "top" program is a top program, regardless of competitiveness of the field.
 
What do you mean by the southwest? Arizona and New Mexico have only 3 programs between them, Colorado's only program is in Denver, and the southern California programs are mostly in LA/San Diego. UCLA is probably the most competitive of the bunch, and I wouldn't really count that as southwest. One problem you may run into with child fellowships is that they often fill up with recruits from their own general programs: UW has 5 spots but in my year, only 1 spot was open to external applicants and the following year, none. In AZ/NM, I was most impressed with UNM's general and child programs.

I moved from OK. I wish to go back somewhere close by. I think it will be hard to get position in OK as the OUHSC has their Adult Psych residency and they might take their own applicant. I'm not looking for any top tier program.

Do I need to get LOR from Child & Adolescent attending? or Just Adult Psych Attending's LOR will Suffice?

Yes, you should have a letter from a C&A attending as well as your adult attendings.
