CAP Matched but better spot is open

Unless you can convince the NRMP to grant you a waver, you must report to your matched program and work at least 45 days. The other/better program will be looking for someone to start July 1.
 
anon account here for obvious reasons.

I matched lower in the child and adolescent psych match than I had hoped on my list and the program I matched with didn’t fill. As it happens a program that I applied to and would fit well at but that did not offer me an interview has an open slot they are looking to fill that did not fill in the match.

do I have any options here? I’m scared to even mention this to the program I matched at as I don’t want to get off on the wrong foot. Do programs grant waivers here? It’s hard because I don’t want to get a waiver and then not get the other spot somehow and then be really stuck but i don’t think the other program would consider candidates without a waiver.

could I conceivably leave 45 days in? I don’t know if the other program would take me or what happens then

do people transfer fellowship between first and second year of fellowship?

I wouldn't leave without knowing for sure that places still have open spots. Chances are that the "better" spots that you're interested in may be filled or just won't be filled, so they may never consider you to begin with.

I agree with Hallowman above.

Also: There were about 50 open slots and 24 unmatched people. This somewhat "desirable" position is going to be:
1) Trying to see if there are any good unmatched people who can start 7/1.
2) Looking for internal/known people who didn't go into the match that they can rope in. Maybe an internal candidate who wasn't going to fast track that they can throw a bone to to fill.

The didn't interview you, so they weren't in love with your application. Maybe they are used to not filling and only want to train people they love. Maybe they have someone in mind already. No matter what, you aren't worth a violation to them and they won't wait 45 days just for you if there is someone else they like.

Sorry to break bad news. Of course, you can send an email after 45 days, but I wouldn't count on this working and you may burn bridges.
 
The match has good and bad things about it, but it is better for most people to have it than it is to not have it. The unfortunate truth is that it has to be binding or it falls apart, so it is binding. I think 45 days was as unbinding as they dared, but it is long enough to generally keep people where they match into.
