I'm starting internship at Cincinnati in June, but I'm already thinking ahead a bit to fellowship. My clinical interests include Neo and Cards, and my research interest is cardiovascular. On the interview trail I started to learn of a combined Cards/Intensivist option, especially since many ICUs are splitting their patients into cards and non-cards groups. Are there any combined fellowships as of yet? Any programs setting themselves apart? Thanks for any help.

i believe there are combined programs, but i am not sure which ones...i am positive that most large programs have optional fourth year fellowship opportunities in cardiology, such as interventional, echocardiography, fetal, electrophys, congenital heart disease, and of course cardiac icu...if you browse the websites for various peds cards fellowship programs, they may tell you if they offer fourth year fellowships, and if so, in which areas of cards

yes, there are some combined icu/cards programs. its a 5 year fellowship so it saves u a year. i havent heard of icu being an advanced fellowship (a 4th year) anywhere, but then again, there are a lot of places! i think 4th years are limited to EP, echo, fetal, preventive,and cath.

I should clarify. The advanced fellowship of which I speak is cadiac ICU, not pediatric ICU. Several programs have advanced training in cardiac ICU (eg, see the Children's Hospital of Boston cards fellowship website). The training in the fourth year I believe is focused on the management of cardiac problems in the ICU, such as severe heart-failure and post-op cardiac care, etc. I do not believe you are trained in other areas of pediatric ICU if you go this route. Programs with large enough hospitals to support an independent cardiac ICU would be most likely to provide this training.

There are three routes into cardiac intensive care:

1. PICU fellowship with 1 additional year of cardiac ICU training (4 years)
2. cardiology fellowship with 1 additional year of cardiac ICU (4 years)
3. combined PICU/cardiology fellowship (5 years)

Several of the larger centers offer the additional year of training, ie UTSW in Dallas, Arkansas and Boston.
