Sorry to hear about your hand problem. Here are my thoughts:
1. Get to an appropriate specialist (hand surgeon, neuorlogist) to get diagnosed, specific treatment and any recommendations regarding doing surgery (Ignore this is you've already done all this).
2. Make sure you are using proper ergonomic technique. The most common mistakes I see are flexing the wrist, having the elbow flexed excessively. Get one of your surgery faculty to watch you to see if there are any adjustments you need to make before any bad habits become ingrained.
3. Try different needle drivers. There is one with a ratchet lock instead of rings that's supposed to help with hand pain (I forget what it's called). Try palming vs. fingers in the rings to see if one is better.
