Pick something you are seriously interested in.
Choosing a specialty for potential income 5+ years from now is a recipe for dissatisfaction.
It looks like you just gathered and listed today's highest paying IM fellowships. They are not really all that similar as professions in terms of bread/butter.
Which are you actually passionate about? If you can't answer that yet, you have more work to do -- choose electives, shadow, etc accordingly.
Since $$$ seems to be a big motivator for you, instead of looking for big annual numbers, keep in mind you will make more overall doing something that pays 250k for 20 years (you enjoy it, therefore not trying to retire ASAP) instead of 300k for only 10 years (you're miserable for a decade and striving to retire ASAP).
A note for reflecting: Keep in mind you could die any day.
Which field would be a more enjoyable way for you to spend your time?

PS: To answer you initial question, when you apply for fellowship, the committee will be more interested in what you did during residency than a 222 Step 1 and no med school pubs. Do well on the other steps, start doing research in a field you are actually passionate about, go to an academic residency program with home programs in XYZ, be proactive about matching into XYZ -- it will improve your odds.
