Changing of the guard

I'm so relieved that Intern year is over...best of luck to all the new R1s that are about to suffer, and to the rising R2s who finally get to do what they set out to do. 🙂

This is a misconception. You needed to get the PGY-1 year because you will see a lot of IM on the neurology floors (cardiac disease, HTN, DM, ID, renal disease, rheumatology, GI bleeds, skin rashes & oncology not to forget geriatrics), so this is also what you set out to do. If you took your IM prelim year as "something I needed to do while suffering" you aint gonna make a good neurologist.

As for your PGY-2 year, wait till you start it. It is gonna be a lot of suffering....but that is because you need to learn what neurology is all about. It is another year of internship.

I am saying this as a PGY-4 who has been through the mill. If you dont know your IM, you will be a poor neurologist. It is plain idiocy to assume you can do neuro without understanding IM & the prelim year is something you didnt set out to do.

PGY-2 year is no breeze, for me it was even harder than the PGY-1 year. and yes there is a lot of medicine in neurology, esp during the stroke and neurocritical care rotations. Just remember, for neurology you suffer for 2 years not just one.

Geez, people. Give the guy a little credit. Finishing internship is still a milestone, so congratulations.

...now get back to work.

seriously. I don't think he was saying intern year was unimportant, or that things were going to be a breeze from here on out. Just taking a moment to say that he's happy intern year's over and excited to be starting on neurology at last. I'm right there with you, alternatesome1! 🙂

Well at least it's not neurosurg where you suffer 6+ years.

There is NO way most nsurg residents are compliant with the work hour restrictions. I know many that top 110 hours a week easy.

Agreed! Those were a couple of interesting interpretations of a straight forward sentence.

After just finishing a neuro residency, I feel all that IM stuff is true to a point. Yes, you need a foundation in IM, but your knowledge of the finer points of IM will quickly become obsolete. You likely will spend most of your residency as a neurohospitalist, but many of us will pursue life as a clinic neurologist and consultant relying solely on the neuro stuff and not depending on our 1 year of IM training to take care of sick inpatients which should be left to your IM colleagues. So the IM stuff was important but what is to come PGY2 and beyond is way more important. Congratulations on finishing intern year, I am certain you will find the PGY2 year a challenge but a significant improvement over your intern year.
