I was just wondering if anyone knew how difficult it was to switch anesthesia residencies? Or if anyone has done this? The program where I am (I am a CA1) currently has undergone some very negative changes recently which make the working environment extremely malignant. Several major faculty have left he program and they are having trouble attracting new attendings. I am also worried the program is going to go on probation. I really enjoy anesthesia but the working environment had become so hostile I have thought about changing to another specialty. I am not sure how to begin the process and wondered if anyone had any suggestions.

Obviously, I do not want to say exactly where because I do not want word to get out but I will tell you that it is a smaller program in the Southeast.

Ah **** now you got me worried, as I am looking at small programs in the southeast.

I am only applying in the South/Southeast - so I am a little worried too. How about a hint?!
 
I know that several programs leave spots open for these and other types of circumstances. The one that comes to mind is UMASS. The program director, (Dr. Deduch, I believe) openly said this to me. I can't tell you of any other programs off the top of my head, but I can tell you that UMASS was not malignant by a long shot. The downside is Worcester. Contact her, she is one of the most down to earth/nicest PD's I met on the interview trail.

OK Dude/Dudette, heres how you handle this.

If you are committed to leaving, leave in an eloquent way. Tell your program you have family issues and you need to move (to wherever you want to go), or whatever. Strive to leave no mud on your face, since you are not the cause of your dissatisfaction. Strive for a smooth transition.
At programs you are interested in, there is no need to throw mud on your current program. Merely tell them you need to move to their geographical location.
Staying out of the political limelight is a trait that will lead to your longevity.

Changing programs will not affect your career, assuming you are in good standing with your current program.
 
Problem is that this is very, VERY late to be making a switch. With the new year starting this Friday, you will be very hard pressed to find a program that will even be able to do all of the paperwork and licensing necessary to allow you to transfer. That limits your options this year to staying at your current program and trying to transfer as a CA-3 (very unlikely to happen), or trying to come in to a program off cycle.

At this point, I would NOT tell your current program of your desire to transfer unless you can find a program that will accept you. Again, because of the timing, that will be very difficult (For example, the state board of Texas delayed the start of one of our fellows for licensing issues (temporary lincense mind you) by two months).

Thank you for this information. Do you know of any way to find a list of other programs who do this? How do you go about contacting program directors without it getting back to the program I am in currently? This is a tough situation to be in and I hate feeeling like I have to take these steps. But it is getting to the point where coming to work is affecting my life in a very negative way. Both the chairman and the residency program director have recently left this program and they now have interum chairs for both positions, just to give an indication of how malignant this program has become. Thanks for all of the input thus far.

personally I dont think its worth switching programs.. If you feel that the program is so bad that it will affect you when you get out then by all means leave.. I doubt this is the case because no program is that bad.. If you wanna leave because someone pissed you off etc.. they didnt give you fourth of july off etc.. dont leave.. It aint worth it.. the evil you know is better than the evil you dont know.. plus its only 2 years.. so private message me and tell me your situation if you dont mind
