Whoops.............

I'm a little appalled that if the shot was successful he'd be expected to keep playing with the rib fractures. Not like quarterbacks get hit or anything. If one of my factory workers fractured a rib he'd be on all kinds of work restrictions.

does anyone have a picture of doing this under ultrasound with the pleura labeled?
 
I can try and get some tomorrow. I think we will be doing an ESB (not rib block but close enough.)

I don’t know the CPT, but there is one.

Maybe best used diagnostic tool in chronic pain?

I’ve used it to cover an area so I can do another painful procedure (Botox in an allodynic scar or area, Qutenza, etc).

I did two today - and totally forgot to record them. I even told my resident "we have to record these." He reminded me as we were pulling the needle out on the second one.

I'm not blaming them - just saying, we forgot. Recording the image is not part of our work flow so easily missed.

u don’t know the Cpt? So u don’t bill for it? I’m afraid it’s billed too close to a TPIs which frankly isn’t worth it...
 
The ESP is a 64999 right now.
 
Off topic, but why the ESB vs. paravertebral? I've heard of and seen, but never personally did, an ESB.
It's super easy and doesn't drop the lung. It's just not as effective

If you are billing for guidance you must have a saved image

The pleura is very very easy to see under ultrasound. I am also very surprised about this complication, I wonder if anyone knows definitively and guidance was used, my guess is not
 
In other news the Chargers are looking for a new doc.....
