One program description tells that .........The surgery program has 15 categorical residents and graduates three chief residents each year. ........
Does this mean that only 3 out of 15 will graduate. IS this pyramidal program?

Cheif in Surgery,,,,,,,?????? is it final year resident????????

Three people per year; five years. Three x five = fifteen categoricals. In surgery, all fifth years are chiefs.
