There may be a few more, and I am sure more in the works as lots of programs are starting to offer guaranteed admission to their peds program if you match into child neuro (making 5 years at the same place). Most of these are in the midwest, and I think it is a pretty big incentive to look at a program. I only accepted interviews at places that had 5 years guaranteed for a couple of reasons.
Places are adding this option fairly rapidly, some like Pitt, Utah, UMich, don't even have a 5 year option listed on the SFMatch distribution list yet.
When I interviewed at Mich, they said that they had a drop off in the quality of applicants once the match started (I think this is the fourth year of the match) because they had to basically tell people that the odds of you matching here for peds is highly unlikely.
When I was at Baylor they told me that they fully expected that the Child Neuro / NDD training system (2-1-2, with two separate matches) would be revamped as soon as the current contract with the SFMatch expires in a few years.
