Just made me think about a question regarding match. If I rank a place as my top choice, and they also say they want me, is it guaranteed? Or is it like the Draft lottery where the worst team in the league has the best chance of getting the first pick but it's still up in the air?

I ask because I read someone talk about 'a computer deciding my fate' and was wondering how deep that is. Is there chance involved, or is the match set up so that if I want a place above all and they want me in one of their spots, that's where I'll go?

It's basically like this.

You rank your top 10 programs (or 20 or 50 or 100)

And the program ranks its top 10 applicants (or top 20, or top how many spots they have)

If you ranked them #1 and they ranked you anywhere from #1-#10 (if they have 10 spots available), then you will match there.

If you ranked them #1 and they ranked you #12 (and they only had 10 spots) then if at least 2 people ranked other programs higher, you would match there, however if all 10 people they ranked #1-#10 in turned ranked them as #1, you would not match there

Now you'd have to do the same thing with the #2 residency that you ranked and hope they ranked you in the top (whatever # of spots they have).

Rinse and repeat 🙂

Loaded question. While it's designed to take the applicants (over programs) preference... there could easily be a group of people the program ranked higher than you, thus eventually knocking you off.

From the General Residency forum (and ERAS subform), I think it's safe to assume programs say a lot but you should take it all with a grain of salt.

The impression I get is that the computer goes through your ranked list and the PD list to look for the highest matches. So if you ranked them 1, and they ranked you 5 but have 10 spots, then you'll get it. If they have 4 spots only, and their top 4 people matched with them, then you won't get a spot because you weren't ranked high enough. Somebody correct me if I'm wrong?

Okay, so it's not technically up to chance at all (even though it may feel like chance, it's based on a concrete matching system which only incorporates people/program's choices without adding some randomization factor).

Not 100% true (in your last scenario someone ranked #1 at a program ranked that program #2, and they could still match over you if you're #12). The computer simply uses an algorithm to ensure that everyone is as happy as possible. Meaning they try to give the students their highest match (54% match to their #1), and same for the hospitals. So yes, if both you and the program rank each other #1, you're guaranteed to match there. It gets more iffy if you rank a school #5 and they rank you #37 and your first 4 rankings all didn't match.

To be transparent, I know there's nothing I can do about it now but I'm married and can't help but wonder how much of that part of my life will be up to chance, since it won't just affect me.

No, there is indeed some chance, particularly when your ranks differ from your programs' ranks and when couples are involved.

After working with attendings, residents, and med students for the past 5 years, I'm very familiar with this process. Pachewisc is the most accurate: If you and a prospective program both rank each other as #1, you're guranteed that spot. But things get very iffy when a program ranks you #1 and you rank them #10 or vice versa or when an even greater disparity exists. When the ranks are misaligned, the algorithm kicks into overdrive and becomes very esoteric. But as has been said, when we get to that point we're very savvy as to whether we like a program and whether that program reciprocates the feeling, hence the 54% top choice match rate.

Does anyone have a good idea of where they're headed next year? And have any of you guys received financial aid packages yet?

By the end of the month I will know if I got into my state school.

I haven't heard of financial aid packages yet either. Just the sole scholarship offer from Creighton but nothing else.

Hahaha! Nice! I tried to stay away from Metal so I wouldn't freak anyone out. Don't even get me started on Black/Death/Doom Metal or Hardcore. 🙂

Yes, other schools do this, too. GWU does it very similarly.
