With all the talk around here about which school is better, I have heard one consistent comment. The biggest factor, possibly most important are the third and fourth year clinical rotations.

It seems to be difficult to figure out which schools offer good rotation options and which schools have good mentors.

So I am hoping that some fourth year medical students and beyond would be willing to talk about good and bad clinical experiences at various schools.
 
Heard nothing but great things about DMU's rotations.

Yes things change from year to year and many of those posts are old but I believe it can help give some prospective you won't get from a school's website or sometimes even interview days.

Look at each schools' hospital affiliations and see how many of them are 350+ bed hospitals, how many are trauma 1 centers, do most of the school's affiliated hospitals have residency programs or just a few, which residencies do they offer

Best advice. Also good to check out that thread posted above.

I also focused on how many elective rotations were allowed so I could do what I wanted where I wanted. Maybe a bit more work and responsibility on myself but I'd rather these things be in my control.

There isn't really a magic ratio. In my opinion, the bigger the hospitals the better. Just because a hospital is small doesn't mean your education will be bad, though. Nevertheless, I feel like my educational experiences were better at the larger hospitals I rotated at (more patients, more lectures, more residency positions, better medical equipment and electronic medical records).

you haven't read the reviews by few 4th years on DMU's school specific thread.

Im sure this differs pretty significantly from school to school.

Here (attached) is MUCOMs list of Affiliated hospitals. The main teaching hospital, St Vincent's, has over 800 beds and is nationally ranked in 5 specialties or so. Not too shabby.

Some DO schools focus more on rural care, though, and so will have more of an emphasis on small community hospitals.

I think it's important to mention that while it is very good to have "better" rotations (like at bigger hospitals etc), it's also possible to get quality experience at "worse" locations if you are proactive.

I would add to your statement by elaborating on what I think you were expressing, i.e. there are good rotations to be had at both bigger and smaller hospitals.

Advantages:

Big: you see more variety and have more specialties, possible access to PDs and residencies you'd want to impress or explore.

Small: you have more hands on. Sometimes you can even be first assist on your gen surgery clerkship since its just you and the doc.

Small hospitals tend not to be teaching hospitals, so, from my experience, the attendings dont want you to be there.

The experiences of everyone I know have been the exact opposite. At teaching hospitals med students are dogs, but at the smaller hospitals you have the place to yourself and much more responsibility.

I am just describing my experience.

And on the opposite end of the spectrum, I felt like my experiences at smaller hospitals were better overall. Better attending contact, greater chance to do procedures (at a 200 bed community hospital EM rotation, I put in a chest tube, did an LP, sutured like 20 patients unsupervised, etc). At a few of my larger sites (MD predominant sites), I was treated as no more than a medical record request faxer. For cardiology and medicine I was at a 800ish bed lvl 1 in downtown Philly, and I legit wrote 2 notes the entire time. No interns cared to teach me, let me learn how to present patients, got to know me etc. They were all drowning on their own in tele (cards) with heavy pt loads and very medically complicated pts.....go I cant blame them for not teaching me. I did have a great experience at Lehigh Valley Cedar Crest tho, which is like 1100 beds with TONS of DO/MD residency programs. Very very impressed with that hospital. Anyhow, just goes to show you how site specific things can be.

It does make it hard to determine what's a "good" site to rotate. IIRC, the user Orthojoe said he tried to do his rotations at hospitals that had residencies associated with that specific rotation. I forget where he talked about how he chose his rotations but he certainly did extensive homework on each site before committing to it.

It all depends what stage of the game you are at. That EM elective was my first EM elective (and hell, my second rotation). I wanted to learn how to do these things and how to function in an ER environment.....not necessarily make a name for myself anywhere yet. I wanted to learn. Now that ive decided on EM as my specialty, I am doing electives at big lvl 1 centers where id love to go for residency. Maybe I wont learn as much, but I will get my foot in the door and get myself known. It all depends what your goals are really.

But in my experience, Ive learned way way more at the smaller centers, where the attending's pt load isnt as high, and they think its cool to teach you. At big academic hospitals, the focus is on training residents, and you as a med student fall into the background.

I wouldn't discount hospitals with no or few residencies. My 3rd year rotations were at a 350 bed hospital with only a family practice residency. Had a great experience. I was allowed to do more and put into many more positions where I actually had to think. We were much more experienced than the 3rd year medical students at many other programs and it showed when we went on audition rotations. The third years where I am doing my residency (large center, lots of residencies) don't do jack and it shows. It is kinda sad.

Credit goes to THH. I decided this was so helpful, I would bump it. I also added dates to the posts so people will know how old the info is before clicking on the link. This should also help people know which schools need updated the most.

I also reordered some of them from newest to oldest (some were off), and fixed links so they go directly to the post page, as I had some issues viewing the "jumps" on my phone.
