CMRs through Outcomes MTM

I am a floater pharmacist and sometimes I see these CMR sheets in patient's bags of meds that I have no idea what to do with. The clerks just ignore them when I am working and I am thinking that they just have the regular pharmacists do them later.

I would really like to know what is going on with them, but there are no other pharmacists on when I am working and the techs and clerks have no idea what the pharmacists do with them.

I look at the sheets sometimes when it is slow and I can't figure out what I'm supposed to do with the sheets when the patient comes up to the window. Do the patient's get the sheets after we are done talking. No one that I'm working with knows.

How does the process of CMRs work with Outcomes MTM?

Can someone walk me through the process of doing one of these CMRs? My company isn't much help with training me on these since I'm a floater.

When you say "sheets" what do you mean? What do they look like? I can't really help you until I know what sheets you're referring to.
 
It looks like a cover letter. It says something like "thank you for talking with me on 1/1/15 about your health and medications." Then it has a MAP and PML.

You would need to be able to log onto that store's outcomes which I am 99% certain you can't. If you fill in for certain stores all the time log onto outcomes and add those store's NABP to your account. After that, log in and look at the CMR training, it is all extremely straight forward.

If that's the case then it looks like the CMR was completed and the sheet you're referring to is the patient handout. Just give it to the patient.
