Columbia vs. Northwestern

You shouldn't have problems matching in the northeast from either school, but Columbia will have an edge due to its location and the connections you can make there. When you say you'd feel more supported at Northwestern, is that because your support system is nearby or because something about the school itself seems more supported?

Based only on this, I think any prestige difference between Columbia and Northwestern is negligible/irrelevant to getting you to match competitively in the northeast (or anywhere you want), so I say Northwestern is the way to go.
I'm going to slightly disagree with riv. Chicago is the second largest city in the country and the center of lots of efforts on social medicine/justice. I don't know what connections you are looking at, but I don't think anyone will not recognize Northwestern as a solid program. I also can't believe any desirable Northwestern or UChicago residency candidate (if we're trying to compare with Columbia and other Ivies) would have problems getting into residencies along the I-95 corridor.
