More importantly, why does this combo even exist? The rationale for it's existence seems to be minimal, other than having another slave/resident work a few extra years for low pay.

The only, and I mean only, reason I could see this being worthwhile was if you wanted to do full scope FM at a hospital that only allowed IM boarded doctors to take care of hospital patients.

My line of thought was that it might convince folks that want to do primary care but also want the IM name/prestige.

Purely guesswork on my part though.
