Can anyone shed some light on why med school admissions care about whether you come from a medically under-served area? Where I grew up is medically under-served, and I would say this did impact my or my family's access to health care on multiple occasions, but it hasn't really impacted my education. Are they hoping you'll want to return there to work? Or will be more willing to work in under-served areas? I feel like I'm missing something.

Yes and yes. A person coming from a rural underserved area is more likely to go back there as a physician compared to someone who has lived in an affluent neighborhood their whole life. Experiences from growing up in these underserved areas can also help educate their colleagues in medical school and beyond.
