I know, I know this is likely going to be a neurotic question, but I just finished the PE and I'm sweating bullets over here. I totally messed up on timing on the first one, took me by surprised, missed doing an abdominal exam and asking them patient to return for a followup - got cut off by the bell. The others I did better at, got my OMM in three times, was compassionate, physical exams were a little shoddy but SOAP notes should have been good (aside from missing a secondary dx or two). I had two questions though:
(1) When listening to the heart and lung sounds in the front, I asked the patient to pull their gown down in the front to expose their upper chest. Was that okay or should I have listened to it by sliding my hand underneath the gown? I'm not sure if asking them to pull their gown down to expose their upper chest was too much, some guys basically exposed their whole chest.
(2) Was it okay just to discuss the most likely diagnosis if you were sure? I didn't mention the differentials unless I wasn't certain. I wrote all the differentials down in the SOAP note though.
