Is there a black and white rule to bill commercial insurance like the 8 minute rule for medicare? I've heard it varies from state to state and is all contract based. Opinions and expertise?

I think the 8 minute rule is for every insurance. And it should be. In order to be able to sleep at night, you better be able to defend what you bill.
