Location, local ties, comfort with the familiar, connection with an attending, a gut feeling, clinical mentoring, not wanting to be a little fish in a big pond, no desire to be an academic, etc.
For a big-name academic fellowship, probably yes--but you knew that already. For a clinical fellowship, not necessarily--especially if the community program in question has a well-respected attendings with good connections to their alma mater.
