Your situation is actually very complex. It sounds like an unofficial IA, but I also can't believe that your were put on probation, a pretty big deal, and there is no record of it?

Hi Goro, thank you very much for your reply. In this case, I guess it's fine to not disclose this since it's not an official IA? I did ask the Dean if he would classify this as an IA but he said he didn't know since there's no universal definition of IA, he did say it was an infraction though.

I think that it's safer to actually check the IA box, explain as you did here, and ask the Dean for a LOR explaining that it really wasn't an official sanction and that you've been a good boy/girl ever since. These types of transgressions are not treated very seriously, at least by my Adcom.

EDIT: I just saw DV-T's post, so this makes me further recommend my advice here.

Thanks for @Goro and @DV-T's response. I hope this will not impact my application so much since I'm not a US but CDN applicant in the future so I don't know if it's going to impact the application, considering the pool is already very competitive

I'm sorry for not ID-ing as CDN since this is my first post in the thread but thank you everyone for the advice. I'll follow @Goro's advice and talk to the Dean about this

This is absolutely an IA. You were put on probation, whether or not the school makes a record of it is irrelevant. The risk in leaving it off is having an LOR writer making an indirect reference to it, even in your defense, and red flags start waving. Then you move from a IA to lying on your application

I would say yes. It is an action taken at a level higher than a class/professor.
