Confused about LORs for TMDSAS

this is my first year applying and I am totally confused as how I should send my Lors to TMDSAS. My school has all my recommendations. Should I go to the evaluators and ask them to send them again to TMDSAS with the accompany form?

you have to print out the paper from the tmdsas website and give that to your commitee to submit with your letters.

I printed the form, two pages that needs to be filled out. Should I take this to my evaluator (AGAIN) for them to fill this out and send it. What if they want to send it electronically.
