Confusing Boston University Letter of Recommendation Section

I am sending my LORs to Boston using interfolio. They said to list it in the committee section if I am using interfolio. What do I enter in these spaces?

University: (UCLA?)
The person responsible for preparation of this letter is: (My name?) or Interfolio?

Telephone:? , or Email?

i think my school uses virtualevents or whatever so for that section, i put my university name and for the part that asks who prepares the letter, i put the head pre-med advisor - just whoever is in charge of writing the committee letter.

Just put Interfoloio under the person and leave the names blank. Those blanks are designed for committee letters, which I'm assuming you don't have. It's just so they know where to look for your letters.

I put my University name in that field, then in the First Name, Last Name fields I put the name of my Health Professions Office chairperson (the person who's signature was on the interfolio coverletter) and then put the Health Professions Office phone number and email for those fields.

Are your letters being sent as a packet, through a letter service, or actually individually?

I don't have a committee letter, but my undergrad institution does have a letter service that I'm utilizing. I contacted BU, and they asked me to put the name of the person responsible for preparing the packet underneath the letter service section, and to take out the individual letter writer names. So I contacted the letter service, and asked if they could give me a contact name and info, and they were really cool about it and did so.
