Hello, if anyone registered for 2014 equavelncy proccess in Canada, would you please send me a copy!
I know when i register i can download it, but i need to check with my university first if they will agree to fill it up for me!

Hi! The form has your information and photo so you registering is the only way to get it!
However , i have a question to anyone that is experienced in filling this form.
(POST SECONDARY INSTITUTION ) is it the pre-dental school if i have attended one or is it the same university that i have graduated from....???
