For those of you that work in hospitals... do you see physicians loading Coumadin very often? (vs just starting with the recommended 5mg/d) If so, does your pharmacy say anything about it or do they just let it go?

i see it...when it happens its because the physician was uneducated and the pharmacy will always make a point to say something about it....

Do you guys have any specific guideline or journal article that you reference when you talk to them? I was thinking the education part of it might work better if there was something simple for them to read.
 
I have seen them do like 1 or 2 days and give 10 mg on those days.

The CHEST guidelines for anticoag are a great resource to point people to.
