Creative Thank You

I'd like to thank the podiatrist that I have been shadowing (not to mention a thank you for writing a recommendation letter for me). Any good ideas on a 'thank you'. I'd send a card or flowers or something but I'm hoping to be a little more creative or thoughtfull. Any ideas??

I gave a gift card so that he and his wife could go out, and a letter thanking him for his support and time. But that is not that creative.
