To the docs on here, let me present a couple of scenarios

Scenario #1: Interventional Pain doc (who is also a BC Anesthesiologist) in practice for 3-4 years wants to transition from full time pain to full time OR anesthesia

Scenario #2: Anesthesiologist (who is also BC in Pain) in practice for 3-4 years wants to transition from full time OR Anesthesia to full time outpatient Pain.

Question: who would have a harder time obtaining credentials from a hospital (for anesthesia) or an ASC (for performing pain procedures), the doc in scenario #1 or the doc in scenario #2?

All things being equal, ease of credentialing will likely be case by case weighted to discussions behind the scenes.
In my experience: credentialing depends on the facility's needs, your firm commitment to staff anesthesia per-diem during critical time slots, proven number/case type/insurance mix of procedures that you will schedule, etc.

Btw, Which scenario applies to you?

A hospital is a bigger bureaucracy, so that'll be a tougher. They'll want to see logs, maybe proctor you a bit, etc. The ASC will likely crave your pain facility fee, so that'll be easier.

#2:

Building up capital doing exclusively anesthesia with the goal to eventually start my own pain practice. I had done pain many years ago.

For those of you involved with ASC credentialing : to credential someone like myself to perform procedures like ESIs, MBBs/RFAs/Kyphos/SCS trials etc, do you require a certain number of cases within the last XYZ number of years?

If so, and a doc doesn't qualify for those numbers is he simply out of luck or is there a possibility to obtain credentials if mentored by another doc for a brief period of time?

here: usually just kyphos plus medmal insurance confirming coverage doing them plus peer letters.
maybe you can have a face-to-face with the medical director of ASC you are going to. good luck.
